IMHO, if the injector has toooo high duty cycle rate, and if something happening with the fueling like inline fuel filter is too dirty, the fueling goes to lean...

Could be bad fuel or anything, if it gets less fuel than it should it will melt, doesn't matter how good the map is.

Exactly, and no one knows if there have been any mods done to the car since Debo had it. You surely cannot fault previous owners for something like that.
